This PR resolves #14 given the feedback from TAG that there won't be a well defined "private browsing mode" in the near future (action item http://www.w3.org/2015/10/29-webscreens-minutes.html#action01).

It also addresses #148 to simplify the steps to start a presentation. It make it clearer that monitoring the list of presentation displays and asking user permission can happen in parallel - otherwise implementations that don't use background monitoring won't be very usable.
