In preparation for the June 2019 release, we are going through the repositories to check for broken links and other issues.

This PR upgrades this repo to using the most recent version of the lesson template. It also removes permalinks in the form of /setup/ for instance. We found that using this approach makes the site unusable offline.

It looks like several links in the discuss and reference pages refer to old episodes for which the content has moved around. Double check 5e165c1 to ensure the mapping is still accurate.

There is a link for the entry infective in the glossary that is not defined.

I see lots of contributions where none of this placeholder text has
been deleted, but also quite a lot where the first line has been deleted,
but the 'ext below' hasn't (exactly in accordance with the current
instructions).
Clarify wording of PR & issue templates
Use the same colors but apply 12.5% opacity.

Solution boxes use 30% opacity
Also, improve contrast of comments

This contribution adds reference to the web page of the lesson in the Carpentries and makes it easier for visitors who want to contribute to get familiar with the good first issue badge.
This adds a list of current maintainers to the README.md.
gold looks more like the yellow from the original badge
display it only if both `page.title` and `site.title` are defined.
