Provide initial or system-level instructions as part of extractions

As mentioned in #319 (comment) , it would be useful to provide contextual instructions to the LLM during an extraction. These may include term definitions or acronyms we'd expect to see in the input text, but don't want to keep providing redundantly through the extraction schema.
