This service allows an unauthenticated user to answer questions relating to an incidence of sickness and provide a PDF copy of their answers which they can use as a notification document to tell their employer they want to receive statutory sick pay (SSP).
This form is not processed by HMRC at all and is only provided as a guidance tool as linked to by other organisations.
