When converting signatures to types, an implicit permission type variable was generated for each function type; now only one is generated for the whole signature, shared by all function types that don't mention a permission variable.

Permission elision generated too many implicit permission type variables.
This has a couple of hackish workarounds that need to be removed before it lands, because
withterms are desugared too early, leading to potential name clashes.
