Move bug report template into new folder structure

Description

Currently, when 'New issue' is clicked the issue form does not appear and instead the markdown of the template ends up in the issue body:

• Moved the issue template to the appropriate folder structure based on the guidelines here.
• Fixed a few syntax errors in the issue form yaml markup

After the change the form looks like:
