Simple way to read Google Sheet into a Google Form using Google Apps Script. Creates a simple Google Forms survey from spreadsheet on Google Sheets using checkboxes and limiting to one response per user.

usage

1. Create a Google Sheet

2. Add checkbox options to column A, one per line

3. Go to Tools -> Script Editor

4. Copy Code.gs to the editor

5. Click Run icon

6. Click View -> Logs to see Published and Edit link
