No Audio output to headphones from pcie soundcard. HDMI audio works fine

Bug Description

So this has been going for months now. & it's about time someone made a post about it.
Setting default playback device to HDMI audio makes the audio output fine from the monitor speakers. but it sounds crappy as do most monitor speakers.
However when set to headphones (ie creative SB pcie soundcard audio) there's no audio output to headphones.
tried different XBOX games and different official & unofficial Audio & GPU driver versions to no avail.
Note that this the only app that this happens every other app/game works just fine both from headphones & HDMI audio.

Expected Behavior

No Audio output troubles with any other application tested.
