Bug Fix: Check mark in form

As a user, I should be able to reliably filter based on the form. At the moment, the check mark is somewhat glitchy. I need to double check this functionality and figure out if the state of the application isn't being stored correctly, or if the UI is not reflecting whether or not the box is currently checked.
