"Integrated 720p" doesn't load

The default quality settings seems to be "integrated 720p". On multiple Videos this specific source doesn't seem to load at all. The "split 720p" source loads instantly, as does "integrated 360p". Maybe set prefer_uni_sources to False by default?

There's still a couple bugs with av-merge which is why I haven't made it the default yet. Specifically stalling when a network request for a segment fails. Also stalls due to false determinations that a segment was deleted by the browser.
