Upgrade tutors-publish-html to match Tutors Reader style

Tutors has two generators - documented here, we term 'dynamic' and 'static'. Examples here of the same course in both styles:

• Static Reference Course

The Static version has fallen behind the style used in the dynamic version. Upgrade the static version to be more comparable with the Dynamic version. Explore architectures for the static generator to use components from the dynamic project (challenging!).
