right now, the TOC assumes which titles to generate TOC entries for. Instead, we should allow the user to define which titles to use.

This allows supplemental material to have a TOC

Prior to this change, we were explicitly generating the TOC for the main
content only. This switches it on for supplemental content. It also
generates a TOC entry for every page title on supplemental. This is
not the proper solution long term, but it works today. See puppetlabs#822
