DBError when submitting custom form

Hi,

I have created a custom form to fetch information for an employment contract. When submitting the filled in form, all required fields get cleared and tagged w/ error messages saying that required fields need to be completed.

I checked the DB and found this error:

If I resubmit the form (after once again having populated all required fields) immediately after the first failed one, it all works.

Has anybody got any ideas what could cause this behaviour?

You need this:

• session: Regenerate Session Id #6379

As for the DB error you can drop (delete) the table and run cron a bunch of times. This will recreate it and repopulate it with data automatically.
