The dissertation guide (page 36) states that:

If there is more than one page, place the "Page" heading at the top of subsequent pages. If additional chapters are listed on subsequent pages, also include the "Chapter" heading.

The sample included in this repo does not have enough chapters/sections to overflow onto a second page but more real examples probably will. I artificially added a bunch of sections and verified that these headings are not present when the Table of Contents overflows onto a second page.

This also applies to List of Tables and List of Figures sections that spill over onto more than one page with the respective substitution of "Table" and "Figure" in place of "Chapter" cf. ToC.

If we don't have a solution in the class file, you can always manually add this after the PDF has been generated.

@weiglemc This could produce unnecessary labor if, say, you find an issue in the text, recompile to PDF, then have to manually re-add all of the headings.

A programmatic solution would be preferable here. :-)

See Himarsha's changes to CoS_ETD _Template on Overleaf which produces the desired headers in TOC on subsequent pages for Chapter, Table, and Figures. FYSA, I did note one issue where the header isn't written if there's only one entry on the trailing page.
