Implement "rated by" field with ability to rate on behalf of others

Select from the respondents assigned to a survey or add a custom user name. Will need to talk with the development team about how this will work. The ideal situation is that when respondents add ratings, their name gets tagged, but I think there are situations where people (admins especially) will be adding ratings that other people provided.
