Disallow @title on noteref elements

We have seen examples of title attributes used in noteref element, the content of which were the note text. I think this markup risks double announcement of note text, and that the validator should disallow it. The guidelines currently refer to the Daisy Accessible Publishing Knowledge Base, and the title attribute is not used in their examples either.
