cloudstack currently broken

The cloudstack format was considered broken in #241:

move some broken formats to ./formats-broken to fix later

This is the tracking issue for the cloudstack format.
Note: I created #259, I use kexec myself but not cloudstack but thought it'd be useful to create issues for both anyway (and separately so they can be tackled individually).
