Add chart note to provider app

Our documentation has a guide on chart notes and this is an FAQ for implementors.

The current https://provider.medplum.com/Patient/{id}/encounter path has a questionnaire on it that doesn't make clear to the implementor how to construct the chart note correctly. Ideally it would have a tool that creates the Encounter, ClinicalImpression, and Condition resources correctly in order to support the correct charting per the spec. Appointment and RiskAssessment are less important and business logic dependent.

Let's wait until this is implemented (PR draft is up #4494) - there are a number of details about "charting" that we are working through that will be important for implementing within Provider App
