The current default behavior can lead to confusion when inviting new users, who have too much access.

The proposal is to always require an AccessPolicy on every ProjectMembership.

This is blocked on #1255 - including a small collection of default access policies should make this an easy transition.

Marking this as v4, because this will be a breaking change.
