Per this thread on mediawiki.org

We are having an issue where any time we try to insert an infobox, we get a fatal exception error. It seems that even empty tags will cause this. This is currently rendering the extension unusable.

I'm having the same issue; I'm running Mediawiki 1.36.2 and downloaded 0.3 from this repository as per the instructions. I've also enabled debugging but don't get any further meaningful error info. Is there anything else I can do to provide useful details?
