The UI won't accept the ALT key as an input, but if you launch it via command line or a modified shortcut to assign the codes, it does work. At least it does for me.
For example, I map the three pedals to Left CTRL, Left Shift, and Left ALT using:
biped2.exe -left 29 -middle 42 -right 56
You just need to find a list of the key codes. Here's a tidy one I found: LINK
I'm actually liking this better than using the UI now. It takes a bit more work to set up, but you can make a series of shortcuts to toggle between different mappings for different applications. It remembers the last config on launch, so you don't even need to use that shortcut every time, which is handy if you want to add the main shortcut to your startup apps.
I do wish there was a silent launch option, so it would start up and minimize to the tray automatically, since I don't need to see the UI often.
I could do this with Pedable. For illustrator work, being able to hold modifiers with the pedal is awesome.
