Description

Currently, you can add a custom Skill, but you can't ever edit the name after creation.
Also, when you initially create a Skill, you can't add a modifier. You have to add it after it's created and on the Edit modal.

Proposition:

Make the skill name editable and add the modifier to the initial creating form.
