♻️ Update "Contact Us" Form to Look Like SSW.Website's Form

Pain

As per my conversation with @bradystroud , we will be updating the UI of our "Contact Us" form to make it the same as the one on SSW.Website. This will help us keep things consistent.

Figure: current form is missing information and poor in UI

Tasks

• Add a "How did you hear about us?" field to the form
• Update the UI to match the screenshot below

Hey team, few things:

1. DRY - I would remove the words "Your" from the labels as they're implicit
2. For "Location" I would call it "Region"
3. For "Location/Region" I would make "Australia" default (fewer clicks for most users)
4. For "Location/Region" I would combine "USA" with "South America" as "Americas"... Unless we really have proven benefits in having "USA" as a separate option... I don't think so.
5. I think "Company" could come before "Location/Region" as well
6. I would move "Referral" question to be last + make it optional
7. Do we have a report on the answers provided by "Referral" question?
8. Validation - The validation for "Phone" doesn't seem to work as I can write only letters and it doesn't show any errors
9. Validation - The validation for "Full name" doesn't seem to work as I can write only 1 word and it doesn't show any errors

I agree with all. For location, I'd only change it to 'Region' if we are actually mentioning 'Regions' and not 'Country'.

Can we have the very same form rendered on all sites? As we do with the menu...
This way it's easier to keep consistency when making changes...
