Issue Description

Extension request should not be available in completed or verified tasks.

Expected Behavior

User shouldn't be able to make extension request in completed or verified tasks.

Current Behavior

User is able to make extension request in completed or verified tasks.

Steps to Reproduce

  1. Mark a task completed/verified in my-site.
  2. Try to make extension request on this task.
