It took me quite a while to figure out that for me the extension only works when wikibase namespaces get enabled for semantic mediawiki. It would be helpful to mention this in the instructions.

$smwgNamespacesWithSemanticLinks[WB_NS_PROPERTY] = true;
$smwgNamespacesWithSemanticLinks[WB_NS_ITEM] = true;

When I look at the README having tried this I find that it mentions setting them to false. I did not try that yet. I did not set them at all. That did not work for me. There was no sidebar option to "browse properties". When I manually changed a URL to point to an item no properties where shown at all.

After setting the namespaces and changing and saving an item the data could be accessed as expected. It might be also helpful to mention that data in wikibase existing before semantics are enabled will not be accessible if no change and save has been done for that item.
