Multiple accounts be made with same email

Describe the bug
Despite the unique constraints, it is possible to make multiple accounts and organizations with the same name if you change the case of either. So the unique constraints are case-sensitive.

To Reproduce
Steps to reproduce the behavior:

1. Create an account with pat@netflix.com
2. Create another account with pat@Netflix.com

Expected behavior
A unique violation exception to be thrown.
