Error checks on csv:

missing column
missing team captain
empty

Dealt missing captain, missing column and missing team captain, as well as making sure that the team captain field is 'YES' or 'NO'. Leaving this ticket open because I'm sure there is more we can do with this, including abstracting error handling into separate file?
