In principle it parses the code for the keywords recursively and builds syntax three for "important" keywords like Rational, MatrixForm and etc. For example, when a user type Rational[x,y] or just use hot-key Ctrl+/ on the expression it decorates it with an HTML element, where denominator and numerator are actually a separate child instances of the same cell's editor (not exactly, but a bit lightweight), therefore it allows to make fractions inside fractions inside fractions and etc with full syntax highlighting support and other features.
