[Bug] Cannot tell if feedback form has been submitted or not #96

Steps to reproduce:

Go to Feedback form
Fill in all fields except Feedback type
Submit the form
Get error in the above screenshot
Go back and select a Feedback type
Hit submit again
Expected results: Error message is gone, success message appears, form fields clear
Actual results: Error message persists, no success message, I cannot tell if the form has been submitted
