My idea was to have one form for each location.

What questions or information did I miss?

If you'd like to directly edit it: tell me your Google account and I will add you (or is there a way to make a link that lets anyone with the link edit the form?)

Not sure how the content creation is going, but you could ask if they would like to share examples or facilitate discussion?
Also, if you still go for the persona idea and trying to reach a wider audience, would it be useful to ask if they are academic or not at this stage to be able to plan a bit for the breakout type sessions? Or do you think you can get that from "What do you normally do/work on?"

My hope was that "what do you normally do/work on" would tell us academic/educator/journalist/someone we haven't yet thought of. Do you think it is too subtle/abstract?

Added the "share examples/facilitate discussion"! Cool idea.

I think "what do you normally do/work on" is quite vague and I'm quite sure I be able to answer it without clearly being mapped into one of those categories. I think these answers could be really hard to classify, but I also get that you don't want to ask directly for one of those as this will be too narrow.
@RaoOfPhysics any ideas how to "fix" this?
