Is your feature request related to a problem? Please describe

We don't have a good sense of why people stop using Sensei.

Describe the solution you'd like

When Sensei is deactivated, and the Enable usage tracking setting is checked, show an exit survey modal similar to the following screenshots. Note that although the screenshots show checkboxes, they should actually be radio buttons:

  • The Submit feedback button should be disabled until an option is selected.
  • The Skip feedback button should always be enabled.

When submitted, a sensei_plugin_deactivate event is logged in Tracks with the following properties:

  • reason - Reason plugin was deactivated
  • details - Value of the associated textbox, if applicable

Additional context

As an example, Elementor has implemented an exit survey on plugin deactivation.

This should be added to the 3.5.3 release, which is in the release/3.5.3 branch.

alexsanford1 I don't think it makes sense to do that since WCPC is already part of Sensei with WooCommerce Paid Courses.
