Enable some survey questions to be asked once per order, rather than per ticket.

Supplementary to the per ticket questions allow some questions to be assigned to the order itself under the first name, last name email.
Questions such as contact telephone number, and in our context, Pupils Name and Form Group.
They could still be copied down to each ticket within the ticket data and exports, but forcing entry on each ticket is not needed.
