What is wrong with the following:
num_qubits = int(np.ceil(np.log2(X.shape[1])))
qc = QuantumCircuit(num_qubits, num_qubits)
qc.h(num_qubits)
I get this message qiskit.circuit.exceptions.CircuitError: 'Index 16 out of range for size 16.'
