Adopt issue forms

Motivation

• Make it easier to collect necessary information and improve the issue submission experience by offering pre-filled fields with information about the environment, versions, etc.

Describe the solution you'd like

• Configure issue forms in templates: https://docs.github.com/en/communities/using-templates-to-encourage-useful-issues-and-pull-requests/configuring-issue-templates-for-your-repository#creating-issue-forms
• Adjust the following templates:
  • bug_report
  • feature_request
  • pull_request_template (not sure if the forms works here though)
• Data to collect:
  • bug:
    • Product: Office Web Apps, Office Online Server (default), Office Online
    • Version: get from https://docs.microsoft.com/en-us/officeonlineserver/office-online-server-release-schedule
    • OS: Win/Mac/Linux
    • OS version
    • NuGet package
• Prefill correct labels/tags (utilize whatever coolness the form engine offers)
• Make the issue submission experience generally more friendly (adjust the text of placeholders, remove misleading or confusing fields
