This has been reported as happening after an item has been assigned to multiple people on separate occasions:

Order is: 1, 2, 3, 4
Item is assigned to Bump websocket-extensions from 0.1.3 to 0.1.4 #2
Order is now: 1, 3, 4
Order is manually reconfigured: 1, 2, 3
Item is assigned to Bump websocket-extensions from 0.1.3 to 0.1.4 #2
Order is now: 1, 4

We are having the same issue. It seems to only occur on GM and Officer role. You can see the auto re-order in the audit log.
