Instead of asking users to press "1" when their call is connected, use Twilio's AMD feature to detect if the call has gone to voicemail or not.

@JackWink looks like it was merged with #33

@sinak, et al. remember that Twilio does not recommend using AMD in production environments as it is still "experimental". AMD normally is only 65-80% accurate. This can increase your expenditures versus the "press 1" model.
