Clinical notes contain information before it happens

What happened?

Clinical notes generated for encounters can contain information that happens later in the simulation. An example of this is a clinical note generated for a well child visit for a 1 year old states that the patient quit smoking at age 16.

Notes appear to "leak" future information, at least smoking status and symptoms.
