Open the "Immunisations form"

Measles shows up with numeric keypad entry, as it should.

"BCG (#2)" (which also shouldn't have a #2, maybe a related issue...) has alphanumeric / text keyboard entry.

It may be related that there are two concepts we are using with the name "BCG" - one is a coded yes/no used in a select_multiple, and the other is this one.

That's the case with all the immunisations fields in that History section except for Measles, which is the only one that appears to be working correctly.

Giving them all unique names fixed the problem, though this is still a bug.
