Refreshing notifications takes a long time

I click it and it looks like it works the first time, and then if I click it again it doesn't give any indication of working a second time

tested on newest Firefox on macOS and pixelfed.social

It works for me, but it takes a long time (10-15 seconds) -- what might be happening for you is that the "refresh notifications" action takes so long that it is indistinguishable from the case where it works only once. Possibly the code is inefficient?
