Have you tried Railway? They have an open source kickback program: https://railway.app/open-source-kickback

The demo is now just reporting back username and password when using demo/demo.

The listed demo now gives a 404 error.
I have hosted a slightly-modified version at https://novamps.com/ohmyform as a demo.
Note: this only works on IPv6 and is on a $3 server, so don't expect much.
