When publishing a form, the title isn't visible on top of the fields, and can only be shown as a welcome screen.
A simpler, and less confusing approach, would be to copy the actual Google Form layout, and have the form title clearly displayed in the beginning.
Here is an example of the expected behavior, with the form title clearly displayed at the beginning.
Here is the actual behavior, with no mention of the form title, whatsoever.
Make a simple form.
Cloned latest git revision, then ran docker-compose up.
