Form title should be visible to users

What's this issue about?

When publishing a form, the title isn't visible on top of the fields, and can only be shown as a welcome screen.

A simpler, and less confusing approach, would be to copy the actual Google Form layout, and have the form title clearly displayed in the beginning.

Expected behavior

Here is an example of the expected behavior, with the form title clearly displayed at the beginning.

Actual behavior

Here is the actual behavior, with no mention of the form title, whatsoever.

Steps to reproduce

Make a simple form.

Environment

Cloned latest git revision, then ran docker-compose up.

About the Feature

I would like to see the form title clearly displayed at the beginning of the form, without having to use a “Start Page”.
