I'm currently looking for that too,
I guess a combination of #16 (comment) and https://github.com/markdown-it/markdown-it/blob/master/docs/architecture.md#renderer will do.

This would also work, with the same domain check.

I'm tried to use https://www.npmjs.com/package/@gerhobbelt/markdown-it-attrs for this and it seems to be a good solution.
