Publication footnotes

As a content editor I want to easily add footnotes to my publication.

Footnotes: initial thoughts is the they could make the single page view more complicated (assuming we want them at the bottom), though I'm sure we could work it out.

If we add multiple text paragraphs with footnotes on one page the footnotes are listed after each paragraph rather than gathered together at the bottom of the page.

The automatic numbering restarts on each paragraph.

I've tried the patch - it does work! Each paragraph of text here is a separate paragraph entity.
