Service type: Add modal

Is your feature request related to a problem? Please describe.
User wants to add Service type through a modal window.

Acceptance Criteria

GIVEN I am a logged in **ADMIN** user
AND I navigate to Organization settings > Components tab

WHEN I click on the Add Services button

THEN I see a modal window to add:
- Service type (required)
- Description (optional)

I also see option to "Save", "Save & New", and "Cancel"
