Not all swatches are being generated

I have a file where the color inventory is not generating all the colors I'm using. Two files attached - one shows that I'm using EEEEEE on a layer, then I generate the color inventory and EEEEEE doesn't appear. Any help would be appreciated... I've been trying to get this to work all morning and it's driving me crazy. :)

Just started a fresh document and can't seem to get the symbols or the colors to generate. "No Colors found :(" is this part of the same bug?
