sql: PRIMARY KEY to support range queries #58

The underlying walker does support range queries, but the planner does not understand how to set the lower and upper bounds, like:

CREATE TABLE t1 (x INT, PRIMARY KEY(x));
EXPLAIN SELECT * FROM t1 WHERE x > 10;
