Section 5.7 discusses the pre- and postprocessing pipelines. There should be a hint, that $transform-original is usually a bad choice for modular DocBook, because no XInclude processing has taken place on the source file at this step.

For modular DocBook, you should try to avoid $transform-original and use transform-before instead. If this is not possible, you can parse the source file with Saxons -x switch to enable Xerces (see discussion for Issue #455).
