Issue

When answering a form more specifically matrix or multiple-choice questions on the mobile, the UI have no hints of further options leaving users unaware of all options.

Suggestions

Either break the matrix into multiple vertical multi-choice questions or make through UI clear that the table is not navigated fully in the page. Below couple of inspirations.
