Donations page can be more robust

Currently, we are asking for people to donate via Paypal.

This is a simple click away, but I am thinking we can make this flow richer. Maybe with a lightweight form that we can capture their email, name, the bridge they are part of, or some kind of note that they would like to send.

This can also help us with future outreach, and give us a sense of who our donors are.
