https://github.com/biosimulations/biosimulations/issues/new/choose doesn't have basic option #4775

If you click on 'New Issue' from https://github.com/biosimulations/biosimulations/issues , it takes you to https://github.com/biosimulations/biosimulations/issues/new/choose , which lists a bunch of options. none of them are a basic link to https://github.com/biosimulations/biosimulations/issues/new which just lets you report an issue the way you want; there's only very complicated forms to fill out.

I can't say for sure that the information it asks for is not actually helpful, but I suspect it might be--the form has the air of 'list a bunch of things we think will probably be essential' that turn out to not actually be essential after all. Certainly as a user, I had almost gotten to the point where I would fill out the form with nonsense, then edit the issue and delete everything and just explain what the problem was. Then I discovered I could just remove the extra stuff from the URL itself, and that https://github.com/biosimulations/biosimulations/issues/new would take me to a blissfully-simple form.

I think those choices should be revisited. Maybe we don't need those forms at all; maybe they can be listed separately from a 'just report something' link that just goes to /new?

(Also, the sample text in the 'bug' form ('A bug happened!' and 'I loaded the page!') is kind of cutesy, and more annoyingly, doesn't go away automatically when you click in the form; you have to delete it first. It's a small annoyance, but a consistent one.)
